TPH School Immunization Program at Bond on February 15

Nurses from Toronto Public Health will be in the school to provide free vaccinations to students at Bond on February 15 according to the schedule of TPH School Immunization Program.
Please find attached the information and consent package for the catch-up vaccine clinic to take place at Bond Academy, which are intended for students and parents/guardians:

This catch-up clinic will take place during school hours, and will offer Meningococcal, Hepatitis B and Human Papillomavirus (HPV) vaccines. These vaccines prevent cancers and meningitis, protect students against serious infections, and are usually offered to students in Grade 7. However, during the COVID-19 pandemic, some students did not receive these lifesaving vaccines, especially those students currently enrolled in Grades 9 and 10.
If you are missing any doses of these vaccines, you can still catch up! To get vaccinated, be sure to hand in your signed consent form if you have not already submitted it. The consent form must be completed by a parent or legal guardian. You won't be able to receive these important vaccines at the school clinic without a signed consent form. On the day of the clinic, remember to wear a loose sleeved shirt, eat something before the clinic, and bring a mask. Also bring valid ID or a health card and your yellow immunization card, if available.

As a reminder, secondary school students that are aged 14 and older can provide informed consent to receive a vaccine in the school setting. Please note, students who want to be immunized must provide a signed hard copy of the consent form. Electronic consent forms will not be accepted by Toronto Public Health.
